The Passenger Locator Form Prague is used by public health authorities to locate the passengers traveling from abroad to the country. The information provided in the form helps to locate the travelers in case they get affected by some disease during their travel. This PLF is just a form and not a visa.

The information to be filled in the form includes personal information, information about the country you are traveling from, address of previous stays, health records, etc. The form is to be filled by everyone even if they are traveling in a group or as a family. Children of age six or above are also required to fill in the form.

Categories of countries based on risks

During the pandemic, the countries are categorized as low, medium, high, and very high risked. If a traveler is coming from one of these areas, they are supposed to get PCR tests or show their vaccination certificates. Only travelers from extremely high-risked countries are not allowed to enter the country.

Who are the Low-risk countries and what are the rules?

The low-risk countries include Canada, Qatar, Hong Kong, Jordan, Australia, Korea, Hungary, New Zealand, Taiwan, Vatican City, Poland, Saudi Arabia, Singapore, and Macau.

If you are coming from a low-risk country, you do not need to undergo PCR tests if you are fully vaccinated. The travelers from these countries also do not need to isolate themselves once they reach the country.

Who are the Medium-risk countries and what are the rules?

The medium-risk countries include Portugal, Andorra, Denmark, Spain, Finland, Italia, Iceland, Malta, Luxembourg, Slovakia, Sweden, Netherlands, and Monaco.

Travelers coming from these countries are not required to undergo PCR tests if they are fully vaccinated. In case they are not vaccinated, they are required to hold negative PCR test results held 72 hours before the departure. Also, they can show negative antigen test results held 48 hours before the departure.

Who are the High-risk countries and what are the rules?

The high-risk countries include Switzerland, Greece, San Marino, Norway, Lichtenstein, Ireland, Estonia, Belgium, Bulgaria, Croatia, Cyprus, Latvia, Austria, Romania, and Germany.

Fully vaccinated travelers do not need to undergo PCR or antigen tests and also they are not required to isolate themselves. Unvaccinated travelers coming from high-risk countries need to show the negative test results of the PCR test taken about 72 hours before the departure.

Very high-risk and extremely high-risk countries

The rest of the countries in the world are either very high at risk for not being listed amongst others. Travelers from countries of extremely high risk are not allowed to enter the Czech Republic. Only if they are residents of Czech, and they are coming from extremely high-risk countries, they are allowed with either a vaccination certificate or negative PCR test results.

The unvaccinated travelers from very-high risk countries also need either a vaccination certificate or a negative PCR test result. The PCR test must be held 72 hours before starting the journey.
